I.e characteristics indication atleast 3-5 .
Eg : specific location, specific sensation, specific agg and amel factor , concomitant, thermal, thirst, craving, aversion, discharge, sleep position, side Affinity, tissue or organ affinity, history of Suppression, history inheritance disease

Match Patient *Available* Characteristics indication with Homeopathic Remedy as per Physician knowledge of Materia Medica
Potency Selection:-
– more characteristic indications show strong vital force needs higher potency and less repetition
– Less characteristic indications show weak vital force needs lower potency and frequent repetition
Potency Selection Based on Experience of physicians
Beginner : less confidence more frequent repetition.
Experienced: less frequent repetition
Master : One dose wait and watch… With SL
Based on Patient Case
Acute :
– if characteristics indicate 30 , 200 , 1M water dose stat dose or repeat as per need mostly use water dose
– if only common indication then needs therapeutic use means more physiological dose i.e low potency like Q , 6c 12c etc in BD, TDS ….till improvement starts then stop medicine and wait and watch with SL.
Chronic :
– if Characteristic indication matches exactly: higher potency like 200, 1M stat dose and wait and watch with SL.
– if mostly common indication then need therapeutic use means more physiological dose i.e low potency like Q , 6c 12c etc in BD, TDS ….till improvement starts then stop medicine and wait and watch with SL.
Caution:
– Don’t stop suddenly any life saving allopathic drugs like Anti Epileptic, Anti Hypertensive , Hypoglycemic agent , Blood Thinner , cardiac medicine, vasodilator , anti psychotic , anti allergic etc…means if we stopped suddenly may cause sudden worsening of patients suffering….
– if possible coordinate with consultants regarding the decision of stoppage of medicine..
– you can tapper down these medicines once your remedy acts well and is able to manage transition or ask patient consultants to tapper down allopathic medicine.
– Steroids always stop in Tapering manner
– Always Check Govt Guidelines as some disease conditions are mandatory to be treated under allopathic guideline eg Tuberculosis..but you can provide supportive management
– always be safe in medico legal aspect
Now we move to discuss follow up Management
Once we Prescribe and the patient comes for follow up and says kuch bhi fayda nahi hua always check the initial consultation paper then ask each complaint he said in initial consultation and ask improvement in percentage mostly then patient realise haa isme to 20% improvement hai isme 30% improvement hai , are haa sleep acchi Hui hai , pahle horror dream aate the ab nahi araha , mood accha ho gaya hai , bhook acchi Hui hai , stool saaf ho raha hai , urine ki burning bhi kam hai etc….
If No improvement at all do recase taking
– if Totality suggests the same remedy then upgrade potency for eg if previous lach 30 then upgrade to lach 200..
– if Totality suggests a different remedy then Prescribe a different remedy eg if previous totality lach and now Sulph then prescribe sulph ..
If Improvement seen
– stop medicine, it’s actions and vital force reaction start , continue with Placebo and wait and watch till improvement stop then give next potency and wait and watch.
if improvement is seen a new set of symptoms appear
– check whether any recent exciting causes behind new symptoms like error in diet , lifestyle, family,society, environment etc if any then correct it ..no need change medicine but in spite of correction no Relief then recase taking and decision based on totality.
– if your remedy works well in a chronic case but sometimes acute appears…wait and watch if not resolved then recase taking make acute totality Prescribe accordingly once this phase gone do recase taking then if totality same as constitutional remedy prescribe last give same medicine if totality change give according to current totality.
If our constitutional medicine works well but as his history of asthma in winter agg and currently this time in winter also aggravates what to do , how to understand our medicine is working or not ?
– Check whether Frequency or severity is reduced or the same ..
– if reduced that means medicine is correct
– if not reduced that means medicine is incorrect..
– Need to change medicine based on new recase taking and current totality..
– if improvement is seen but recurrent then think anti miasmatic remedy
if Improvement is seen but whenever the remedy stops improvement stop , and reappear symptoms
– Increase potency
– if again and again failed that means either palliation occuring : Do recase taking if remedy change give new remedy if remedy same then think about anti miasmatic block give anti miasmatic remedy based on current case.
– Nosodes like medorrhinum, tuberculinum, syphilinum, Tellurium , Baccillinum etc or Polychrest remedies like Sulphur , merc sol , thuja etc are anti miasmatic remedies.
– after antimiasmtic 1M stat dose wait for a few days then re- administer the same remedy which was a well indicated remedy but failed.
If there is an improvement seen but appearance of new symptoms like rashes, itching, urticaria , acidity, Coryza etc
– Check whether similar conditions are suppressed by allopathic or other non Homeopathic treatment in past..then it’s a good sign or recovery sign.. educate patients about recovery signs
– if there is no past history then check any current Ailments which may cause this …and accordingly think about the new prescription if totality change
If the overall physical general improves, the feeling of energy but the chief complaint remains how to manage?
– Educate patient about real recovery… explain we support your vital force if your cold cough goes away in 10 days with the help of our remedy your vital force able to remove this in few days
– if patient is impatient and wants quick relief in cold cough headache fever etc..then auxiliary mode of management like honey tea , warm water gargle , warm fomentation, cold fomentation, steam bath , steam inhalation , haldi doodh , massage etc as per need Or some specific mother tincture or biochemic remedies to give accessory support till main remedy able to fully active vital force
